• If you are hoping to get your K-6th child placed on a 2026 spring NWC sports team, this is a required form. 
• Filling out this form does not guarantee your child a spot on an NWC team.
• However, unless this survey is filled out, the athletic dept. does not know your child wants to be placed on a team.
• WINTER sport seasons are closed and practices/games are underway!
